This is really strange. I assume you have followed the instructions
here: http://tortoisesvn.net/node/191
(especially that you have run the ClearInstall.exe tool).
Also make sure that you have properly uninstalled any previous TSVN
version (check the 'Software' panel in windows).

If you then installed the latest TSVN version and you still have the
same problem, fire up the registry editor and check if the following
keys are there:

Key: HKLM\Software\Classes\*\shellex\ContextMenuHandlers\TortoiseSVN
Value: String: {30351349-7B7D-4FCC-81B4-1E394CA267EB}

Since you have the context menu for folders, we can assume that the
shell extension itself is registered correctly, but the entry for files
seems to be gone/corrupt/wrong. If that still doesn't help, then I have
to admit that I'm out of ideas...
