You can find the required files in our repository:
http://tortoisesvn.tigris.org/svn/tortoisesvn/branches/1.6.x/contrib/issue-tracker-plugins/

username: guest, leave password empty.

Have a look at the txt file, and the required files are in the inc/ folder.
