Could it be that the dialog autocloses?
Under TortoiseSVN->Settings and General->Dialogs 1 there are some options to
control autoclose. I have "Always close dialogs for local operations"
checked, which makes the Add-dialog to be shown very briefly (only flickers
on the screen) but then closes automatically.

And suggest me to send a bug report to the TSVN developers after automatic
closing? I do not think so untill this is an issue in TSVN to show an error
dialogue before automatic closing...
