Looks very good!
Small changes though:
* use "OK" instead of "Accept" for the buttons
* the first dialog misses a "Remove" button
* maybe instead of the checkbox "relative to", just use a label "url
format" and then have "relative to repo root", "relative to ..." and
"absolute url" in the combobox.
