Keep the 16x16 icons separate and copy them into the .ico file manually. This is not done every day, so it should be acceptable.

I painted the big icons first, scaled them down to the smaller versions and touched up all 16x16 and many 32x32 icons manually to get clear edges.

The scaled down version of 16x16 icons can only be used as a hint to the icons shape, nothing more.

On thing that could also cause fuzzyness is that the brighter parts of the icons are transparent (they only have about 60% opacity, which makes the underlying icon shine through. Is this intentional?

BTW: I'm missing my tick mark that extends into the file's icon :)
