If you're running TSVN on Vista, you should consider using a nightly
build from the 'latest' directory.
http://nightlybuilds.tortoisesvn.net/latest/

The reason is that there are some problems we have already fixed, but
can't be merged back to the 1.4.x branch for various reasons.
TortoiseSVN will work properly on Vista when version 1.5.0 is released,
until then, you should use a nightly build.
