FWIW, I see a similar thing. If you already have a copy of explorer
open, you'll see overlays in file dialogues immediately. Otherwise, the
first time you navigate to a location that you'd expect icons, they'll
be missing until you refresh or navigate to a different folder and back.
Interestingly (to me), this seems to be the case whether or not
TSVNCache is running (?!).

Of course, I also find that overlays won't be displayed in certain
non-standard file open dialogues, such as those in Visual Studio 2005,
ironically.
