I don't remember there being a long discussion either so I did some
searching. Those titles were changed in r4559 on 5 Oct 2005. AFAICT
there were not a large number of users asking for it, and all I can find
is this reply to Thomas Hruska who was driving Stefan nuts on that
particular day:

Somewhere in there Thomas asks for the dialog titles to match the
context menu entries. I think the only person who asked for this was
Thomas himself, and I agree with Stefan and Jody that this is an
unhelpful use of space. I would prefer to see either:
Add : TortoiseSVN - %path%
or
Add - %path% : TortoiseSVN
Looking at my own taskbar, almost all the apps put the document title
first and their own name at the end. It may not be MS-correct, but it is
certainly common practice.
