How about an 'Other commands >' last entry in the level2 menu that
will hold the lesser used commands. You could start with a fixed
division between level2 and level3 commands and only allow the user to
promote options to level1?

That would depend on the overall usage of commands (and yes, there
probably would be lots of requests like 'move this to level2, move
that to level3). But IMHO this would be a good starting point.

No chance for a tree-view with drag-drop support to let the users move
the available options between the three folders right :-) (you know, a
bit like Internet Explorer allows you to organise your favourites?)

Or maybe three option list areas above one another? (I know, ugly if
one needs a scroll bar and the others don't).
