[TSVN] Doc patches

Hi

Attached are two patches to the documentation. I created them from the top
level of the WC.

One details how I fixed the problem I was having the other week with
permissions. It might be too verbose. But it's not mentioned in the docs -
for reference, I thought that using SSPI meant commits would happen as my
account, not Apache's, which is what threw me...

The other inserts a warning (is there a <caveat> tag?) into the Apache
server setup section, suggesting that the admin sets up an account for
Apache to run under, as opposed to defaulting to local system (when
installed via the install package from httpd.apache.org).
