I get that as well. In my case the reason is that I also have
TortoiseCVS installed and there simply aren't enough icon overlay slots
available in Windows to cover the TSVN 'Added' overlay, so it shows no
overlay.

The problem is with Windows, not TSVN. But we do not fail as gracefully
as we might.

Ideally, if there are not enough overlay slots, we should substitute a
different overlay, eg. 'Modified'. But I don't know if we can even tell
that Windows has not honoured the request for a particular overlay.
