My feeling is that "old" and "new" don't not have (the right)
connotation (for german speakers: die richtige Assoziation).

What about:

Create patch
     using base:
     to:
Apply patch to:

or

Create patch
     starting with:
     ending with:
Merge (patch) into:

I would prefer the second (maybe use "Apply patch to" --- but then
there's no mention of merging).
