So you'd need a sort of shell-shell extension that registers the icon overlays, then calls both TortoiseSVN and TortoiseCVS for each registered icon (overlay)?

order of importance seems about right. Just "deleted" should come
before "added".

There are only 12 slots available, three of the 15 slots are used by
Microsoft for e.g. the 'link' overlay, the 'shared folder' overlay,
...

And believe it or not: there are other programs beside TCVS and TSVN
which use overlays. So we're talking about a situation where TCVS and
TSVN simply _can't_ coexist on the same computer without one of them
loosing overlays.
