These problems are very hard to diagnose.

I wonder if we should add a *load* of stats to the shell extension - how
many filenames it's looked up, how many times it's hit the cache, etc,
etc. I'm not sure if these need to be persisted somewhere, or if just
keeping them 'per-process' would be OK - perhaps this would depend on
whether people have the shell set to start a new process per folder.

I'm quite happy to go through and add these - are there any suggestions for
an easy way to display them?
