These fields have not been on the dialogs for years. There is an FAQ
with a good explanation:

Also, Subclipse does not provide a PDF of documentation. Our
documentation appears within the Eclipse Help system, for example
click the Help icon on the dialog you mention. The docs are also
available online:

http://svn.collab.net/subclipse/help/index.jsp

The screenshot looked correct when I just glanced at it.
