Did you ever say what version you are using? If 1.1.5, then did this ever
happen before 1.1.5?

Generally speaking, what the adapter does is outside our control and if
these problems are occurring at the file system level, the adapter would
have to try to fix it. If we are issuing an incorrect command, or if
there is a threading issue where we are issuing a command while another
one is still running, then it might be something we are causing and can
fix.
