It sounds like your status cache is out of sorts. Refresh has always
fixed it for me, although sometimes it takes a Team -> Update. I do not
know how to definitively delete the cache so that it will rebuild, but I
think it is located in the file:

workspace\.metadata\.plugins\org.eclipse.core.resources\.projects\PROJECTNAME\.syncinfo

You can try deleting that file.

There is nothing you can check, you'd need to produce a reliable recipe
for creating the problem.
