My colleagues and I are also experiencing this behaviour sometimes, but
we couldn't find a way to deterministically reproduce the bug.

The problem is exactly the same as described by Carlos:
- local working copy is in sync with repository, e.g. samplefile in
revision 100
- doing local changes
- successfully comitting the local changes: samplefile in repository has
revision 101
- local working copy has samplefile in revision 100 (old revision, old
content)
- update of samplefile required to get back the latest revision
