I found this and was wondering if this is a patch to the subclipse, as
this is something similar we are planning to do.

Don't you typically discover extension points by looking at the
plugin.xml and the tools that are provided by PDE? Perhaps you did
and you are asking because you do not see any that are relevant.

We do not have any extension points of this nature in the product.
Most issue trackers have chosen to integrate via the bugtraq
properties. In Eclipse, there is an arguably better option which is
Mylyn. If you provided a Mylyn connector for your tracker you would
get integration with Subclipse and CVS for free. User's could group
their commits into changesets where the changeset is a bug from your
tracker, they could click on references in commit messages etc.

I'd recommend you start by looking at those two options.
