IMO, for the book, the best reviews are comments, rather than contextual
diffs. Diffs are just too hard to apply, especially when the book keeps
getting tweaked. But rather than put <!-- comments --> into the xml
files directly, I think it might be nicer to enter them into a separate
file as plain prose, like in a 'doc/book/REVIEWS' file, or maybe at the
bottom of 'doc/book/TODO'.
