Minor documentation fix

Hi.

This is a minor documentation fix for a sentence that seems to be a bit
confusing to me for two reasons:

1. There is no file notes/repository_upgrade_HOWTO, but it is called
   notes/repos_upgrade_HOWTO.

2. What does the word "located" mean in this context?

How about the following trivial patch?

   * CHANGES: fix file name (notes/repository_upgrade_HOWTO ->
     notes/repos_upgrade_HOWTO), remove obscure word "located"
