Following up to my own post again...

Not only had I configured with worker, I had adjusted httpd.conf to
restrict the number of threads. This was intentional, I was
investigating how it would behaved. I was using Apache from the end
of January (I think) when I originally restricted it like this and it
worked fine. The same configuration with todays Apache provokes the
delays, but they go away if I revert to the default configuration.
