[PATCH] fsrefactor fire cracker 2

Sorry for being such a slug getting this out.

I have posted an updated patch and log for the remaining portion of my
refactor patch and another increment. This increment moves opens,
closes, etc into the bdb directory. Some of the code is temporary and
will be removed when the vtables go in. I believe both patches to be
current with 5197. They both have been "make check" ed.

The docs have not changed. I'm going to start improving them soon.

If you have any questions about the over-all direction look at the "BIG"
patch. It should help. Once I get to the later increments I will
present a couple options that should clean up the API level indirection
a little. I've been giving some thought to the server side config issue
as well.
