all done with the license fixes

Since I touched so many files, you'll probably need to do an "update" to
merge the changes into your working copy before committing your own work.

But... all done now. Every .c and .h file now has the exact same text
somewhere in the file (the script checks for "somewhere", but I haven't seen
it anywhere besides the top of the file :-)

I did not check any files other than .c or .h files.
