I've been able to isolate the source of this based on a number of similarly reported problems. I am able to accurately reproduce the problem on a XP, 2003, Vista and Win 7 ... The source of the problem is "realtime virus protection" but can't quite pin down what, exactly, is happening.

I can report that deactivating realtime virus protection seems to enable things to work without failure or generating new corruption.
