Ok, before I tried this I also uninstalled TortoiseGIT and TortoiseHG on the off chance they could be causing the issue. I uninstalled TortoiseSVN as well, then rebooted. Then re-installed TortoiseSVN and tried setting the options the way you suggested. At that point our WAN link went down :) !! That is not your problem for sure. It just came back a minute ago and I was able to test and unfortunately it still exhibits the same behavior. I have Filemon logs showing the activity without the one shell extension enabled and with enabled and there is a huge difference in the file read activity. I can post these here if that would help.
