It doesn't you know - top of my file menu is "ATI Catalyst control
Centre", then I have the open/edit/print items... Tortoise is right in
the centre, between the Trend AV scan item, and Winmerge compare items.
Try a tool like
http://www.safe-install.com/programs/context-menu-manager.html to manage
the menus. If you use something like RegMon (from sysinternals) you can
see what it's doing.

Subject: Windows Explorer File menu
I know this has been raised before but TortoiseSVN DOES royally get in
the way of expected Windows Explorer behavior by placing itself at the
top of the File menu. However, it is the ONLY program I have ever
installed that works in this way. If you insist on having it in the File
menu how can I relocate it to a position below "New"? This is a matter
of being a good Windows citizen and makes the program significantly less
desirable.
