Upgrade destroys install

Everytime I have upgraded a machine regardless of version, my existing
install of tortoise has been destroyed.
After the upgrade I have no shell icons on my existing working copies, and
if I create a new working copy
I wont get any icons on those either even tho the right click menus are
there.

Because of this happening on multiple machines Ive had to stop using
tortoise. Uninstall reinstall does no good. Does anyone know of
a way to fix this?
