I suggest the following approach:
* uninstall TSVN
* reboot (important step, don't leave this out)
* install TSVN again
if that doesn't work, check the registry keys under
HKLM\Software\Microsoft\Windows\Current
Version\Explorer\ShellIconOverlayIdentifiers
You should see seven entries there named 1TortoiseSVN-7TortoiseSVN (or
something similar named after TSVN). If you don't have those there, the
installation was not successful.
If however you have more than 12 entries in there (not necessarily from
TSVN), then you have too many icon overlay handlers installed and TSVN
simply doesn't get a slot from the system.
