Patch: Improved margins for HTML manuals

Hi,

Attached patch increases the text width for the single and multi page HTML
manuals. For IE the text is adjusted across the browser window regardless of
its size. In FF and Chrome, the text flows between 800 and 1200 px (showing
a scrollbar for smaller windows and increased margins on wider windows).

Not tested on other browsers.

BTW: the styles.chm.css defines an html body width of 640 (line 349), but
when I view the .chm the text is always adjusted to the full width of the
CHM window (tested on Vista 32). Is the setting relevant for chm files?
