Sorry I don't know. The Microsoft HTML Help compiler is a quite old animal. It can't handle Unicode, which is the reason why you have to change the output encoding of the xsltproc transformation using <xsl:param name="htmlhelp.encoding" select="'windows-1256'"/>. Please check if this is the correct value. I found it by googling.
I know it is necessary for Japanese and Chinese too, but these are not RTL languages.

Have you seen proper arab chm help files anywhere? Perhaps you can ask the developers of this software how they did it.
