Yes, the commit was done several weeks ago. I cannot work out what causes it
because I don't always notice when it re-appears.

Basically, all I ever do is Add or Commit files. Nothing fancy at all.
Ocassionally I will delete but not very often. And yes, everything is done
via the shell extension.

I thought it may have been during an UPDATE but there is one machine that
shows the problem and it is never updated since it is the root development
machine. I have several machines showing the same symptom (which only ever
perform updates and no commits).

Whenever an update is perform, it is always for the HEAD.
