Double "TortoiseSVN" entry in Windows 2000 / XP Start Menu

Using TortoiseSVN RC2, when I right-click on items in the Windows XP
Start Menu's list of frequently used programs or any shortcut that I've
"pinned" to the Start Menu, the TortoiseSVN context menu item is in the
menu twice.
I've also seen this happen for shortcuts in the Programs folder of the
Windows 2000 Start Menu but I can't seem to reproduce the problem
consistently.
I've attached screenshots of the problem in XP and 2000.
