I just did.

"Callouts on graphics
The DocBook DTD has a mediaobjectco element that is designed to
associate callouts with a graphical image. Unfortunately, this feature
is currently not implemented in the DocBook XSL stylesheets."

I might use a much simplified workaround, just a list of links in the
same order as the menu.
