What to do; start WinDbg when it hangs or leave it running always?

Sounds likely..
I wouldn't bet on me being able to do it again, but on the other hand
it's happened a couple of times now.
Maybe provoke it by calling it a bunch of times in parallel manually?

Did I mention that my box is SMP?
