Looks good now.

After killing the cache I get this:

[2112] Pipe WriteFile failed
[2392] InstallEx
[2392] ::Install
[2392] cache loaded from disk successfully!
[2392] PipeThread started
[2392] InstanceThread started

Followed by a long list of requests and file names. And the overlay
icons dislay correctly.
BTW, TSVNCache shows 4 threads now.
