Is it awkward to maintain? I had just assumed that since the Package
Explorer view was refreshed dynamically when any resource was
modified, that the pending operations view would be able to get the
same sort of notifications. But I haven't got a clue if it is able
to work that way at all.

I find the pending operations view convenient, am I the only one?
Just as 'svn status' doesn't list version controlled resources with
no changes, I like having the pending operations view without having
to search through a massive tree of resources manually.
