I do change files outside of eclipse, but most of the projects haven't
been touched in about a month anywhere. And I recreated my entire
workspace and freshly imported the projects into eclipse and the issue
still exists.

My only thought may be that it's something that started with the .30
release. as I'm not sure off hand which release was working and when it
stopped working.
