[RFC] Obliterate - FS transaction design

Hi devs.

I'd be extremely grateful for a little review from anyone with an
interest in Obliterate or experience of the repository FS and
transactions.

I'm designing proper on-line obliteration, while at the same time
bearing in mind that we may choose to implement a simpler (probably
server-side, off-line) version initially because full on-line support
could be quite hard. But I want to use as much as possible of a "proper"
transactional on-line design from the outset, as I think it will provide
a good base for any implementation.

Here is a design for a new FS transaction type that obliterates a
node-rev in an existing revision rather than creating a new revision.
Please read it and give me your reactions. Thanks!
