Yes, people do that. They don't do it because they are stupid, they do it
because the mail clients (including Mozilla which is what I use) have bad user
interfaces that do not indicate that the message currently being composed after
hitting "Reply" has a special relationship to another message. The message
composition user interface looks identical to how it looks if you start a
completely new message.

The real solution is to fix the user interfaces of the various mail clients.
