OK, let's add a line-number attribute to the entry element and state that
entries might come out of order. This will be useful when we change the
blame algorithm to work backwards through the revisions. Then it can
output lines as soon as the revision is known. (I want the API to be
changed in this way as well, to allow for a better UI where blame info is
filled in as it is received.) This prevents another group of users from
being lazy, but we shoudln't allow all our users to be lazy, should we?
